Request a COLOR Review

You do not need to have every detail, or use a particular label for what happened. Start with a short, general description and tell us whether contact is safe for you.

This is a request to be considered, not a public report. A minimal request is enough. Do not include files, private records, Social Security numbers, or children's names or identifying details. An optional account in your own words is available only after you confirm you are an adult.

Before you share

  • You may report your own concern or a concern involving someone else. A family member's request does not give consent on another person's behalf.
  • You can choose no contact. Only give an email address or phone number if using it is safe for you.
  • Safe-contact choices cannot guarantee privacy on a shared device, in browser history, or in an email or phone account someone else can access.

Submitting a request does not guarantee acceptance, an investigation, a response, or legal representation. This page does not publish your description as a public allegation.

A guided first contact

Choose what is safe to share, one short step at a time.

Checking whether requests are available. The form remains closed until availability is confirmed.

You can read the guidance even when collection is closed. Choosing a topic does not send information.

You can begin with the basics

  • Provide a short, general description and the state or local area. It is okay not to know exact dates or names.
  • Say what kind of review or information would help, and whether any contact is safe.
  • A request is considered before any further step. A receipt does not guarantee acceptance, an investigation, or a response time.
  • If there is immediate danger, seek local emergency assistance if safe. This page is not emergency dispatch or a real-time threat-response service.

Submitting here does not automatically notify the FBI, law enforcement, or emergency services.

Five short steps, at your pace. You can go back before submitting. Unknown and unsafe-to-share choices are valid. All answers are for private intake review, not a public report.

Your guided review request

First, your choices and safety

You can leave this blank. Do not identify another person without their permission.

You can make a minimal request without confirming your age. The optional narrative is available only after you confirm you are an adult. Your age choice and whether the concern involves a minor are separate questions. No guardian is contacted automatically.

No contact and unsure both mean no proactive contact. If contact permission is unchecked at review, COLOR must not contact you regardless of any address or number entered. Without safe contact, individual follow-up may not be possible.

This does not alert emergency services or create an urgent-response guarantee. Seek local emergency assistance if safe when there is immediate danger.

Already have a receipt?

Use your reference and private token for a limited status lookup.

Receipt lookup is unavailable until the connection check is complete.

Receipt lookup

After a successful lookup, you can stop future contact or withdraw from further review. These choices do not promise deletion of records already held. Do not submit another description merely to change contact details.
